What this site is

PSIRF.co.uk is an independent reference guide to the Patient Safety Incident Response Framework, written for the people who work with it: governance leads, patient safety specialists, and quality and risk staff in NHS-funded and regulated organisations. The aim is a plain-language account of the framework that stays current and cites its sources, free to read, with nothing gated.

The site is a community benefit project managed by Vatix, providers of safety, risk and compliance software. Vatix funds the site's upkeep because its customers work with PSIRF daily and kept asking the same questions; publishing the answers openly seemed more useful than answering them one at a time. This site is independent of, and not endorsed by, NHS England.

Editorial policy

  • Content is based on NHS England's published framework and supporting guidance, cited with dates on each page.
  • Every page states when it was last reviewed. Claims we cannot source are flagged in the text rather than asserted.
  • The guide does not take advertising, does not publish sponsored content, and does not accept payment for coverage or links.
  • Vatix is mentioned where relevant, once per page at most, and always identified as the site's manager. Editorial content is never shaped around its products.
  • Corrections are welcome and acted on; use the form below.

How the content is maintained

NHS England updates PSIRF guidance periodically, and the framework's supporting documents have moved several times since 2022. Pages here are reviewed against the current published guidance and re-dated when checked. If you spot something out of date, the fastest fix is to tell us.
